This can be described as Co-Production - working together with health professionals as an equal partner, deciding together how best to achieve goals.
In some programme areas, one organisation may be put in charge of the care patients receive from different health professionals – this is known as an integrated pathway hub.
However, that organisation will always be accountable to the Clinical Director for the quality and cost of the care it provides to patients.
This sets out the agreed health and wellbeing outcomes that the patient wants to achieve and how the budget will be spent to help the individual.
They are looking at clinical outcomes, the cost of services and making sure they are of the very highest quality and meet your needs – and they will take your views into account.
The CCG are aiming to work in collaboration to co-produce our local offer by working with our patients, partners and staff in Oldham throughout 2016/17.

For children and young people with special educational needs and disabilities (SEND), the personal health budget is likely to form part of their Personal Budget for their Education, Health and Care (EHC) Plan.
